a) your Nominated Treating Doctor (NTD) and any other treating practitioner who (by agreement) is deemed necessary
b) appointed return to work coordinator of your employer
c) referring agency; and
d) persons/organizations engaged by Rehabworks

Except as stated above or as otherwise required or authorised by law such as state workers compensation legislation, we will not use your personal information, collect your personal information from, or disclose your personal information to any third party, without your prior knowledge or consent.

Rehabworks will ensure that data held specifically in relation to the provision of its services is up to date and accurate and that reasonable steps will be taken to ensure the health information it holds is kept securely from loss, misuse and unauthorized access.
